Co-Op
Why aren't all games Co-Op? I can't think of a singal game that would have been worse with your friend by your side, and at least a thousand games that would have been better with Co-Op? It wouldn't take more than a month to incorperate an extra character into an already finished game.

Some games I would love to see co-op in. But some games just have such a perfect level of difficulty, or stories that would never be able to fit in a character for the protagonist that they would never be able to fit in a second player.
As far as the difficulty of implementing it, the game very well might need to be completely rebuilt. Different story arches, separate paths, agro systems, ammo regulators, weapon dispersion, I mean in a real co-op game (I'm not talking Halo 3 style here) the implementation of a second player is very game altering. So really saying that there is no single player game that wouldn't benefit from co-op gameplay is pretty damn ridiculous.

Co-op is fine with a friend by your side, because then if he starts shooting you or otherwise getting in your way you can smack him upside the head and be done with it.
Online co-op, though, is different. People can be jerks and there's little you can do about it beyond taking care of them yourself (making the entire co-op play not only the complete opposite of how it should be but also, in some games that would require the co-op, impossible to finish). In FPS's that's stopped by simply allowing a choice between friendly fire or turning it off entirely and not allowing the players to change it, but for many other games it would be difficult to stop players from being complete and total dicks. And I don't know about the rest of you, but I find that 90% of the people whose games I end up joining turn out to be exactly that.

I've complained about this for a long time too. Like Final Fantasy 9 was the only FF game to have co-op, imagine games of that caliber always having co-op. Or games like Medieval Total War, allowing more than 1 civilization at a time. Games that exclude you from other people, usually have very little replay value, because other people make games more challenging, interesting, and most importantly fun. PC's have this problem big-time, not everybody wants to buy a copy of some damn game to play with their friends.
Some of my favorite games of all time, are games I played with my friends: Perfect Dark 64 Mario Party series Mario Kart Halo series NHL Games Madden Games. I find sports games to be really boring w/o playing vs or with people. Against computers with the hardest setting, its only a matter of time before I find that play that cant be stopped. Or read the computer's blatent "thought" patterns. There's really no surprise, it's too predictable, meaning a decently intelligent human being, will not have a challenge.
